Question 1) I would choose option two and offer to show him how the app works. The more knowledge he has the better. If he did not work for him then I would suggest something else maybe, but I would take the time to ensure he understands what he is doing.

Question 2) I would choose option 3, to discuss it with the medical team. With all of the information he can then make an informed decision, following from medical advice.

Question 3) I would choose option 1. Again, the more information that Charles has and can access, the better. The more he knows about his condition, the more likely he is to notice changes etc and if or when he may need medical attention.
