    Collaborative Discussion Questions for Bibi Adams:

    I would check that the catheter is draining well first, to ensure his condition is stable. Then I would assess the patient and then based on the results of the assessment I would create a care plan and explain to Bibi what will happen to him in his stay at the hospital. Finally I would contact the wife.

    1. Assess patient first —his blood pressure is a worry (5LITRES aspirated!!)
    2. Check catheter
    3. Discuss plan for 24hrs with him and reassure re childcare/family/wife
    4. ring wife with his consent

    I will focus on my patients first , his vital signs needs to be monitored especially BP , whilst doing that I will keep a close eye on catheter to see how much is draining per hour and that will give me an idea if the catheter is draining well enough or he’s retaining . I will also discuss with him the plan for the next 24hrs , also keep him informed about his health and what’s going on but reassure him based on what I know and the plan for the next stage … I will ask for a consent from Bibi if he’s ok with me contacting his wife .

    I also agree with the comments made, assesing the patient and considering pain nausea and other complications that could arrise. check catheter and once his condition is established and maintained its important then to explain and reassure, check in on mental health, needs of him and his family and with consent contact his wife.

    I agreed with Zara also, will consider gaining more information about his condition before feeding back to him and family. As it looks like he might need further tests – blood, scan to roll out the possibility of enlargement of the prostate…

    1, Focus on assessing patient first: I agree with what Hannah, Forlefac and Zara have said about priortising patients condition first, unless it was a complete emergency I would not contact the family first and even then I would ask the patient if he wanted his next of kin informed. Nikki raised some important observations that I agree I would also undertake, to gather as much information as the patients state as possible.

    2, I agree with Dan that after assessing I would concentrate on the catheter, its drainage to ensure patient hydration. And to check the catheter is patent however like Alex and Jo said this could be a part of the assessment process. I would document this in the patient’s notes and also on the fluid balance chart.

    3, Give him information about what will happen in his 24hour stay in hospital, again I agree with Zara, it seems the logical step to make after the first two assessments, that way the patient can be kept informed, which would aid in the shared decision making processes, maintain automny and justice.

    4, Call his wife to inform her of Bibi’s admission. again I agree with Alex, Jo, Dan, Zara and Nikki, the last process would be to inform the family members/next of kin. Like how Dan stated I would gain consent from the patient prior to doing this. I think that leaving this as the last step, you as a nurse have a clear picture of the plan of actions for the patients care. So when the wife does eventually visit both the patient and the family can be informed about the next processess.

    I agree with Daniel, Priority will be to call BIbi Adams wife to inform her of his admission and make sure I put her at ease as possible. I will then focus on assessing the patient first. I will gather as much information about the patient as I possibly can professionally and record it. Checking catheter and making sure that it is draining well is very important. Then, I will give him information about what is going to happen in at least 24 hour stay in hospital. I will definitely agree with member of my team on this matter.

    I agree with Zara. Firstly I will focus on assessing the patient, then I will check the catheter and make sure it is draining well. The next step would inform hwhat will happen in his 24 hours stay in hospital. In the end, I will call his wife to inform her of Bibi’s admission.

    1. I would focus on assessing the patient first, checking observations and vital signs.
    2. I would then check his catheter , checking its not causing him any pain or discomfort, and that he is draining fluid well, check colour etc.
    3. I would explain the situation to Bibi and what will be happening whilst he is in hospital. What the next step would be etc.

    I agree with Zara, his BP is quite low which raises some concerns but may be due to the 5 litres of fluid that has been aspirated. I would focus on getting his BP up so he is stable, check catheter and use a fluid balance chart to monitor intake and output, discuss with him if it was ok to inform his wife of what is happening.
