For Question 1, I would go for option 2, as if Charles is unable to use the app correctly, he may mis-record information and therefore the data gathered from it may be misleading. Furthermore, by offering to help Charles and support him to monitor his own condition you are empowering him to manage certain aspects of his own health and recognize things that may be triggering his symptoms as he logs them, which can be discussed during the next appointment. This could also be useful in encouraging Charles to engage in his own treatment plan and maybe stop smoking.
For Question 2, I would follow option 3 to ensure he isn’t putting strain on his heart, however cycling may be an important part of feeling healthy for Charles, exercise is shown to be beneficial for mental health. He might also be part of a riding group or go cycling with a friend which, if stopped, could have a negative impact on his social health. Furthermore, we don’t know how often he cycles, how far he goes or even if his cycling is a vigorous activity, her may just slowly ride along. All these factors need to be considered to ensure Charles remains healthy and can keep to a routine that works for him. We don’t want to stop him from doing something he enjoys, but maybe the medical team will advise based on his normal habits and what they feel is an appropriate compromise to maintain Charles health.
For Question 3, I would follow option 1, introducing him to the FAST acronym (Face, Arms, Speech, Time) but I would also reassure him that (based on task 2) his risk of stroke score is low, and that the Rivaroxaban he is on should further reduce that risk as it helps to prevent blood clots from forming. I would also advise that if he is concerned about having a stroke, then maybe he could talk to friends and family and educate them about his condition, so they know what to look out for as well.
