Q1: I would choose Option 2 and look to demonstrate the app for Charles, as this will give me an opportunity to build my therapeutic relationship with him, as well as tailor my advice to his specific needs. I would also incorporate Option 3 into my advice, requesting that he familiarises himself with the app for next time, and if he still finds it difficult, we could consider further alternatives.

Q2: I would choose option 3- it would be important that Charles follows specialist medical advice regarding his exercise routine and the pressures on his heart. I believe it would be important to encourage Charles to follow a healthy lifestyle routine- which includes exercise- where possible, as healthy lifestyle has known benefits, however it would be important that the stresses on his heart were properly managed.

Q3. I would choose Option 2, direct Charles to further information, including NHS websites, information pamphlets, as well as the FACE advice. As Charles lives alone, we could discuss Charles using Personal Alarm Service, so that he can call/press for help if he suddenly feels unwell. This would add an extra layer of protection for Charles, and hopefully some reassurance. We could also look through Charles’ lifestyle together to advice him of anything that could exacerbate his risk of stroke, but also reassure him that at present his BP, RR etc are in normal levels.
